Description: The Executive Director Clinical Integration and Care Transitions assesses, develops and implements continuum of care services, quality and cost data management systems, and utilization management to deliver superior clinical, financial, and operational coordination and alignment across the Network. The Executive Director Clinical Integration and Care Transitions is a strategic leader in programs supporting the transition from a fee-for-service, volume-based care delivery system to a value-based care model with a particular focus on JPS connect patients and across multiple payors such as Medicaid, Medicare, and Medicare Advantage. Success for the Executive Director Clinical Integration and Care Transitions will be measured and defined by the degree in which the Network is able to implement effective and efficient a) population health programs across outpatient-inpatient-post acute care settings, b) total cost of care initiatives, and c) accountable care type opportunities. This position requires close collaboration across multiple disciplines with both internal and external partners, and must align tightly with outpatient, emergency department, and inpatient nursing/operational teams to promote a safe environment of care and facilitation of effective discharge planning.
Typical Duties:

  1. Implement and integrate compliant clinical processes and procedures to support an effective and efficient medical management infrastructure.
  2. Identifies and evaluates potential ACO, CIN, or similar virtual opportunities in order to build innovative partnerships and programs that deliver value in new ways..
  3. In conjunction with the Chief Financial Officer, Chief Quality Officer, and other executive team members, provides insight and direction to achieve exemplary value-based care outcomes in Network patients, including but not limited to development of unique operating and management models, ensuring CMS and payor program compliance, monitoring financial and quality performance, and providing guidance on JPS connect services.
  4. Serves as a thought leader on CMS Innovations and payor strategy in conjunction with finance, providers, and operations.
  5. Works with key stakeholders to draft KPIs and KAPIs for the Network and providers that appropriately reflect quality, stewardship, and cost effectiveness.
  6. Identifies opportunities to improve readmission rates, length of stay, avoidable days, linkage to primary care, appropriate clinical documentation, coordination between services, and other measures as appropriate.
  7. Creates aligned processes for transitions of care throughout the Network inclusive of the hospital, post-acute care partners, patient centered medial homes, Acclaim Physician Group, and other relationships.
  8. Tracks local and national trends to maximize Network success in current or future shared savings programs as well as identify new opportunities for growth. Shares relevant emerging trends, challenges and opportunities.
  9. Directly responsible for case management, care coordination, utilization review, social work services and discharge planning, including but not limited to process planning, recruiting, hiring, supervision and evaluation of staff.
  10. Accountable for leading best practice work across the Network as it relates to care management, continuity of care and clinical outcomes of all case management and social services in both inpatient and outpatient settings.
  11. Coordinates duties with appropriate staff to meet operational program needs and ensures compliance with state and federal requirements, Medicare guidelines and accreditation standards. Develops and implements policy and procedures.



    Required Education and Experience:

  • Master’s degree in Nursing, Business Administration, Healthcare Administration or related field of study from an accredited college or university.
  • 7 plus years of experience in Quality, Care Management or Care Transitions.
  • Required Specific Licensure/Certification/Specialized Training:

  • Current licensure by the Board of Nurse Examiners for the State of Texas or proof of reciprocity of licensure between the State of Texas and another state.
  • Experience using Microsoft Professional Office Suite, including Word, Excel, Outlook and PowerPoint.
